Japan Introduces New Requirement: Tourists Must Have Health Insurance

As reported by Travel Weekly Asia on July 16, 2025, the Japanese government has yet to officially launch its new regulation mandating health insurance for foreign tourists. This initiative response to the mounting costs of unpaid medical bills incurred by visitors. Proposed by the Ministry of Health, Labor, and Welfare, the regulation aims to alleviate the financial burden on Japan’s healthcare system.

A survey conducted by the Ministry in September 2024 revealed that out of 11,372 tourists who received medical care, approximately 0.8% failed to cover their treatment costs. The total unpaid medical expenses amounted to a staggering 61.35 million yen, or around USD 425,000, a significant strain on Japan’s budget.

In response, the Ministry of Health will collaborate with the Immigration Ministry to enhance screening processes for foreign tourists. These agencies plan to tighten checks upon arrival and share information regarding tourists with substantial healthcare costs. Those who neglect to pay their medical bills may face consequences, including restrictions on re-entry into Japan.

This mandatory health insurance requirement will apply to travelers from all countries. Although a version of this regulation has been in place, it only affected tourists from Cuba, Nepal, Qatar, and Jordan. With this rule, the Japanese government aims to put any concerns arising from the anticipated 36.87 million tourists in 2024 to rest.

In addition to mandatory health insurance, Japan will introduce the Japanese Electronic System for Travel Authorization (JESTA), which is similar to the U.S. ESTA and the U.K. ETA. This digital pre-screening program is designed to bolster border security for individuals entering the country.

Through this system, Japan can enhance entry security and reduce instances of visa overstays. JESTA will roll out in 2028 and 2029 for visa-exempt countries such as Australia, New Zealand, and most European nations. Travelers from these regions will be required to submit their travel and personal documents online prior to their flight to Japan. The JESTA system will implement screenings to mitigate security risks and ensure compliance with Japan’s immigration regulations.
